چکیده

Leprosy is a chronic disease caused by Mycobacterium leprae. Its course is complicated by acute onset of inflammatory reactions which can occur before, during and after treatment of the disease. The reactions are classified as Type 1 reaction or reversal reaction and Type 2 reaction or erythema nodosum leprosum (ENL). Reversal reaction is a Type IV delayed type hypersensitivity reaction while erythema nodosum leprosum is immune complex mediated Type 3 hypersensitivity reaction (Ramesh & Pahwa 2010). These reactions can present with varied manifestations as the disease itself. The treatment of reaction is sometimes more challenging than the disease per se. Here we report a case of ENL in lepromatous leprosy who presented with annular papuloplaque lesions in a Christmas tree distribution. Case Report

Annular bullous lesions with atypical erythema multiforme in leprosy.

Erythema nodosum leprosum (ENL) is an immune complex-mediated reaction that may complicate the course of multibacillary leprosy. Bullous lesions in Type II reaction, though reported, are exceedingly rare. We report the case of a 32 year old female patient who presented initially at our OPD with erythema nodosum. Cutaneous examination revealed impaired sensation over dorsum of right foot and thi...

Erythema nodosum leprosum necroticans in a child--an unusual manifestation.

Erythema nodosum leprosum necroticans is an uncommon manifestation of type 2 lepra reaction, encountered in lepromatous and borderline lepromatous cases of leprosy. We report an unusual clinical presentation of necrotic erythema nodosum leprosum in a 9-year-old boy with no pre-existing evidence of leprosy. The lesions resolved completely following multi-bacilliary multi-drug therapy for 12 mont...

Acute exudative arthritis in leprosy--rheumatoid-arthritis-like syndrome in association with erythema nodosum leprosum.

Erythemi'nodosum leprosum is a fairly common complication of lepromatous leprosy and is variously reported to occur in 30 to 40% of all patients with lepromatous leprosy on sulphone treatment (Buu Hoi and Basset, 1963; Cochrane, 1964; Job et al, 1964). As yet there is no satisfactory explanation of the mechanism of production of erythema nodosum leprosum. Among the many hypotheses postulated, t...

Familial clustering of severe atypical variants of erythema nodosum leprosum in immunocompetent siblings

Erythema nodosum leprosum is characterised by brightly erythematous tender nodules or plaques usually in bilateral symmetrical distribution. ENL can also present in several atypical presentations like pustular, erythema multiform like, ulcerative, necrotic, panniculitic and bullous forms. Various precipitating factors for ENL include stress, pregnancy, surgery, lactation, menstruation, infectio...

Type II reaction without erythema nodosum leprosum masquerading as lymphoma.

Lepromatous leprosy is a multisystem disease that can involve many organ systems, with lymph nodes a common extra-cutaneous site to be affected. Rarely, multibacillary leprosy can be confused with other diseases like lymphomas and connective tissue diseases. Herein we report a patient of lepromatous leprosy with Type II lepra reaction involving lymph nodes who presented with generalised lymphad...
